Wurden die Aufgaben am Anfang fair verteilt?
Were the tasks divided fairly at the start?
The plural subject die Aufgaben takes wurden. In this yes/no question, wurden comes first and verteilt comes last.
Mein Teil wurde geändert, ohne dass ich gefragt wurde.
My part was changed without asking me.
Wurde geändert reports the change; gefragt wurde reports the missing consultation. Both use the past process passive.
Die Anweisungen wurden der ganzen Gruppe erklärt.
The instructions were explained to the whole group.
Wurden agrees with the plural subject die Anweisungen, not with the group receiving the explanation.
Leider wurden zwei verschiedene Versionen gedruckt.
Unfortunately, two different versions were printed.
After Leider, wurden occupies the second position. Gedruckt closes the clause.

Was your name left out by mistake?
The singular subject dein Name takes wurde. Aus Versehen keeps the question open to an accidental explanation.
Die endgültige Entscheidung wurde getroffen, während ich weg war.
The final decision was made while I was away.
Wurde getroffen is the past passive of eine Entscheidung treffen, meaning ‘to make a decision’.
Allen wurde gedankt, aber unsere Arbeit wurde kaum erwähnt.
Everyone was thanked, but our work was barely mentioned.
Wurde gedankt means ‘was/were thanked’; wurde kaum erwähnt means ‘was barely mentioned’.

Wie asks about the way the problem was resolved. Wurde follows the question word, and gelöst comes last.

Reporting past actions with wurde/wurden + Partizip II
This passive reports what happened to something in the past. Use wurde with a singular subject and wurden with a plural subject. The past participle names the action. The person responsible can remain unnamed when unknown or not yet relevant.

Word order in statements and questions
In a main-clause statement, wurde/wurden occupies the second position and the participle comes at the end. In a yes/no question, wurde/wurden comes first. With a question word such as wie, it follows that word.
